Laser Training
AUTHORITY
Arizona Administrative Code (A.A.C.) Title 12, Chapter 1, Sections R12-1-1426, and R12-1-1434.
American National Standard for the Safe Use of Lasers, ANSI Z136.1.
POLICY
- All ASU personnel MUST obtain approval from the Office of Radiation Safety to work with or to access laser facilities.
- Only Class IIIb and IV laser facilities are impacted by this policy. Other laser classes (including laser pointers and laser printers) are not covered by this policy.
- Laser personnel and incidental personnel (as identified by the academic units) shall obtain the appropriate eye examinations PRIOR to assignment for baseline surveillance in the event of actual or suspected eye injury, and when they terminate from the university or transfer to other duties without potential laser exposure.

- YOUR EYE EXAM MUST BE COMPLETED PRIOR TO SCHEDULING FOR LASER SAFETY TRAINING. THE VISION FORM CAN BE OBTAINED IN THE OFFICE OF RADIATION SAFETY.
- PLEASE NOTE: It is the individual’s responsibility to pay for the exam in advance.
- Upon completion of your eye exam, submit your original receipt to the PI's department for reimbursement and bring your eye exam sheet to your class for verification or have them FAX a copy to this office at 480.965.6609. Contact ORS for scheduling of your Laser Safety Training at 480.965.6140. Your point of contact is James Burnette, Laser Safety Officer.
Training is held on the third Thursdays of each month from 11:00 – 11:30 or on a case-by-case basis in ECD room 109. This is for ALL INDIVIDUALS working with lasers.
